Due to the Personal Data Protection Act B.E. 2562 (Personal Data Protection Act) specifying that the data subject has rights relating to his/her personal data as set forth in Personal Data Protection Act so that the Nuovo Plus Co., Ltd. (the Company) can perform its duties as a data controller. The Company therefore has prepared this request form to provide the exercise of such rights under the law.

According to the Personal Data Protection Act B.E. 2562, the data controller is responsible for notifying the personal data breach incident to the Office of the Personal Data Protection. Commission without delay within seventy-two hours since knowing the cause as far as can be done. Unless such violation has no risk that will affect the rights and liberties of individuals, so that the Personal Data Protection Officer can take action in accordance with the said provisions. Please specify and report the details of the breach of personal data as far as you can identify, so that the Company could check and notify the results later.
